Oswego Health offers several ways to remotely access your medical records to include viewing provider notes and future appointments, reviewing test results, and renewing your prescriptions.
By enrolling in one of our patient portals, you will be able to access your healthcare records securely and conveniently from your computer, tablet or phone.
Please note that different service lines have separate portals.

Enrollment questions
The Patient Health Portal offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the internet to help manage and receive information about your health. With the portal, you can:
- View diagnostic lab and radiology results
- View and print health summary documents
- View health information such as known allergies, current medications, recent problems, medical history and future appointments
- Update demographic information (address, phone number, etc.)
- Access trusted health information resources
The portal is a free service offered to our patients.
If you were an Oswego Hospital inpatient and you wish to participate, you may have been provided a temporary username and password during your hospital stay. This will be sent to the email address that we have on file for you. The temporary username and password will enable you to log in and create a username and password.
If you had services at Oswego Hospital but were not an inpatient, you will receive a brochure with instructions on how to set up your account.

Your medical record
Most of your test results will be available in your portal account within 72 hours; however, some sensitive results may not be accessible from the portal.
Your healthcare provider determines which types of test results are able to be accessed through the portal. Furthermore, tests of a sensitive nature are not released to the portal.
Your portal information comes directly from your Oswego Hospital electronic medical record. Please report any medical record discrepancies to the Health Information Management office at 315.349.5551.
No, not yet, but maybe in a future release. Emails submitted from the portal are viewed by nonclinical staff. So please do not use this site to communicate sensitive information (HIV, mental health, work-excuse, etc). And especially do not use this site to communicate with anyone regarding an emergency or urgent situation.

The portal is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given to your health records. You may click the "Forgot password" link on the sign-in page to reset your password online.
You will need to contact the Oswego Hospital Health Information Management office at 315.349.5551 between 8 a.m. and 5 p.m. The office will verify your identity and then provide you with a new username and password.

Technical questions
Oswego Hospital takes great care to ensure that your health information is kept private and secure. The portal uses industry-standard security technologies to protect your information. Registering with the portal includes creating a username and password and establishing security questions. Your username and password credentials are then required each time you log in to the site. It is very important that you keep your password private and safe.
We strive to protect your privacy and the security of your information. While logged in to the portal, if your keyboard remains idle for 30 minutes or more, you will be automatically logged out of the portal. We recommend that you log out of your portal session if you need to leave your computer for even a short period of time.
You need to use an electronic device (computer, smartphone, etc.) with access to the internet and an up-to-date browser (such as Chrome, Safari, Firefox or Edge). the Microsoft Explorer browser is no longer supported by the patient portal.
For your security, your temporary username and password expires after 72 hours of receiving, and it can only be used once. To obtain a new temporary username and password, call the Health Information Management office at 315.349.5551 between 8 a.m. and 5 p.m.
